Passport application options
Adult – General: You have never had a passport before, or your passport has been expired for more than one year and you are over 16 years of age.
Adult – Simplified Renewal: You have a passport that has been expired for less than one year, or your passport is within six months of expiry, and you are over 16 years of age.
Child – General: Anyone under 16 years of age must complete the Child application if they wish to obtain a Canadian passport.
Things to bring to the Passport Clinic:
- Two new unaltered identical passport PHOTOS taken within the last six (6 months)
- IDENTIFICATION: Original birth certificate, Canadian citizenship card
- SECONDARY ID:
– At least one piece of government-issued ID with the name, sex, date of birth, signature and photo of the applicant (i.e. driver’s license).
– If applicable, a government-issued copy of any legal documents that support a change in name (Marriage certificate, adoption order, etc.) - Your previous Passport, if you have one (even if it is expired)
- REFERENCES: Names, full mailing addresses, and phone numbers of two people NOT RELATED TO YOU who have known you for at least two (2) years.
- GUARANTOR: A Guarantor is a Canadian citizen, 18 years of age or older in possession of a valid Canadian passport who has known you at least two (2) years. A family member or person residing at the same address as you may be your Guarantor provided he or she meets the above requirements.
Guarantor MUST bring their Canadian Passport with them. Their passport number, issue date and expiration date is required.
NOTE: We are unable to provide a Guarantor. Your Guarantor will need to be in attendance with you when you complete your application (if required). - PAYMENT: Credit Card, money order, or certified cheque (payable to the Receiver General for Canada).
**NO DEBIT OR CASH WILL BE ACCEPTED AS PAYMENT for a passport**
- Adult (Over 16 years of age) 5 year – $120.00
- Adult (Over 16 years of age ) 10 year – $160.00
- Child (0 – 15 years of Age) – $ 57.00
Child passports (under 16 years of age)
- Two new identical passport PHOTOS (taken within last six months)
- IDENTIFICATION: Original birth certificate, Canadian citizenship card
- SECONDARY ID:
– Acceptable proof of parentage (detailed birth certificate or order of adoption)
– If applicable, all documents (including divorce judgement) that refer to custody of, mobility of, or access to the child - Any previous Canadian travel document (passport) issued to the child, even if it is expired.
- REFERENCES: Names, full mailing addresses, and phone numbers of two people NOT RELATED TO YOU who have known you for at least two (2) years.
- GUARANTOR: A Guarantor is a Canadian citizen, 18 years of age or older in possession of a valid Canadian passport who has known you at least two (2) years. A family member or person residing at the same address as you may be your Guarantor provided he or she meets the above requirements. Guarantor MUST bring their Canadian Passport with them. Their passport number, issue date and expiration date is required.
